You'll be asked to complete this form as part of the appointment making process. You should ensure that this form is completed and returned to the Centre's admin team in Cambridge before your assessment takes place.

Fully inclusive assessment fee

The Anglia Access Centre will charge students, via the DSA general allowance, a single assessment fee, which will cover the work required for the duration of the student's course of study. This will include:

  • additional needs with which a student may present
  • any activities undertaken by the needs assessor or Access Centre to review the student's DSA support, for the duration of the original course
  • liaison with funding bodies, equipment suppliers, HEIs, or other stakeholders for the purposes of review of the student's support needs

Students who present with an additional diagnosis during the course will receive a response from the original centre, in accordance with the above points.

Postgraduate and PGCE students may be eligible for a formal Review of their needs, funded through the postgraduate or undergraduate DSA respectively.

  • All reviews must be authorised in writing by the funding body in advance.
  • All documentation relating to reviews will be logged and available to view at audit.
