You'll be asked to complete this form as part of the appointment making process. You should ensure that this form is completed and returned to the Centre's admin team in Cambridge by the date requested.

We aim to offer an appointment within 15 working days of you contacting us.
The Anglia Access Centre will charge students, via the DSA general allowance, a single assessment fee, which will cover the work required for the duration of the student's course of study. This will include:
Students who present with an additional diagnosis during the course will receive a response from the original centre, in accordance with the above points.
Postgraduate and PGCE students may be eligible for a formal Review of their needs, funded through the postgraduate or undergraduate DSA respectively.
