Nurses and AHP new to, or wishing to work in the Primary Care sector require a structured and supported entry into working effectively in the community. This blended learning module aims to prepare nurses and professionals allied to medicine to work within the primary health care team through:

• taught theoretical and clinical sessions
• clinical placement
• supporting resources on the Virtual Learning Environment

The module will contribute to the achievement of Health Authority pledges and work streams concerning staying healthy, long term conditions, health protection, clinical governance and patient safety.

Practice Assessment Document which you will complete with your mentor in practice. There will be a competency framework which will be required to be signed off to demonstrate your ability and understanding of certain issues/concepts.

Poster Presentation consisting of a case study of a patient looking at; roles and responsibilities of the multidisciplinary team, evidence based care given, legal, professional and ethical issues of the case and the provision of health education/promotion.
